Staten Island Medical Home Visit Services

At Staten Island University Hospital, our medical team doesn't stop caring about geriatric patients after they leave. That's why our Home Visit Services program gives primary care continuity services for homebound patients. Our program coordinator screens potential patients and caregivers with a phone interview, and then medical appointments are made with eligible patients. Our in-home primary care physician services include the following:

  • Assessments of patient's medical needs
  • Follow-up visits
  • EKG and routine blood tests
  • Peripheral bone density measurements to assess for osteoporosis
  • Arrangements for skilled nursing, rehabilitation and custodial care services
  • Social service assessments and care plans
  • Caregiver support

Treatment for medical conditions

Eligible patients may have a wide variety of medical problems or chronic illnesses causing disability. We serve patients dealing with senile dementia, Alzheimer's disease, multiple sclerosis, Parkinson's disease, depression and complications from diabetes. Hypertension, heart attacks and strokes are common causes of disability among our qualifying patients, while others may be disabled as a result of fractures or other orthopedic problems.
